Background Check Procedures

1)      The church pastor secures a signed waiver (Background Check Consent Signature Form) from the individual to be checked and sends that waiver to the district office along with $5 for each individual background check requested.
2)      The district office processes the request and receives the criminal background report.
3)      The district office sends the pastor a copy of the criminal background report.
4)      The pastor makes a copy of the report available to those in the church who have legitimate employment and/or supervisory responsibilities for the individual.  Beyond that small circle the report is kept strictly confidential.  The individual involved also has the right to receive a copy of their personal criminal background check.
5)      To ensure confidentiality the district recommends that a permanent copy of the report not be kept at the local church. The pastor is encouraged to destroy the church’s copy after a hiring or supervisory decision has been made.
6)      The original background report is kept in a permanent locked file in the district office. The pastor may request a new copy of the original report at any time.
